A. To the best of my knowledge when i was in flight training every time there was mechanical failure (something as simple as defective guages to full blown engine failure)whether it kept a plane from flying or not was red tagged and logged in a book.Then god forbid that plane should crash that log book is there to prove any defect had been repaired or if there was a mechanical failure listed as cause of accicdent you could tell if it was a recurring problem or something new.
